How do we change, edit or remove the "activate live" form?

Symptoms

• I need to change some of the information on my Unity Multiplayer live request.

Cause

You have submitted your "activate live" form and now wish to change, edit or cancel it.

Resolution

The live request needs to be cancelled manually by our team.
